This document will offer you a birth plan that your care practitioner will actually look at because it is functions as an agreement between you and your practitioner/hospital.
It offers you the ability to initial what you would consent to, what you will need more information about before consenting or declining, and what you decline. Items covered include, but aren't limited to, blood draws, IVs, newborn procedures (vitmain K, newborn exam practices, etc), and more!
This is the birth plan you need in order to have the most peaceful, well informed hospital birth possible.
While this document is intended for use in a hospital setting, it can be used for a homebirth or birth center birth, too.
